Objective: Elderly-onset rheumatoid arthritis (EORA) is defined as rheumatoid arthritis (RA) with an onset at age 60 years or over. Because of the higher frequency of acute onset, often company with systemic features and the higher disease activity, the diagnosis is very important for the EORA patients. To explore the significance of anti-cyclic citrullinated peptide (CCP) antibodies in EORA patients, we carried out the following study. Methods: Anti-CCP, RF-IgM, RF-IgA and RF-IgG were detected by ELISA in sera from 50 patients with EORA, 30 patients with polymyalgia rheumatica (PMR), 50 patients with OA and 30 health blood donors. All the RA patients fulfilled the 1987 ACR criteria for RA. At the same time, the RA patients were also received the evaluation of DAS28. Results: The specificity of Anti-CCP alone was 94.83%, the sensitivity was 56.25%, the positive value was 85.71%, the negative value was 79.70%. The specificity of RF-IgM alone was 87.93%, the sensitivity was 56.25%, the positive value was 72.00%, the negative value was 78.46%. The specificity of RF-IgG alone was 91.38%, the sensitivity was 40.63%, the positive value was 72.22%, the negative value was 73.61%. The specificity of RF-IgA alone was 91.38%, the sensitivity was 43.75%, the positive value was 73.68%, the negative value was 74.65%. The sensitivity of anti-CCP combined with RF-IgM, RF-IgG and RF-IgA was 100%, the sensitivity was 37.05%, the positive value was 100%, the negative value was 74.36%.The level of Anti-CCP from EORA was positively associated with DAS28 (r = 0.419, P = 0.021). Conclusion: Our results demonstrate that the combination of Anti-CCP, RF-IgM, RF-IgG and RF-IgA is highly specific and moderately sensitive for EORA. Anti-CCP alone is also highly specific and remarkably sensitive for EORA. Anti-CCP is a valuable diagnostic tool for EORA. It might be one of disease activity marker for EORA.
